Information Consent and Terms of Treatment

It is important to gain your consent to collect and release information to treating practitioners and other parties involved in your rehabilitation. 

These could include your treating doctor, specialist or other health professional. 

This is for the primary purpose of providing quality assessment, diagnosis and treatment. 

Please review our Privacy Policy Statement.  I have read the above information and understand the reasons for collection of information and the way in which it might be used. I understand that this practice does NOT operate on terms of an Account basis and I agree to provide payment in full for service provided on the day of consultation.  

For online, telehealth appointments: I acknowledge that no online service is a perfect substitute for an "in person" consultation with a physiotherapist.
